RFU Young Player Registration Form (90kb)
This form should be completed by all players of North Hykeham Rugby Club who have not previously registered with the RFU, are re-registering, or have data ammendments.
Notification of Transfer of a Player Form (14kb)
This form applies to those players wishing to transfer their registration to North Hykeham Rugby Club. The transfering player is required to obtain the transfer authority, so their registration is in accordance with NLD youth registrars instructions.
RFU Notts/Lincs/Derby Application Form (20kb)
This form should be completed by players wishing to be put forward for selection for the Notts/Lincs/Derby County Squad.
Players Protocols Form (4kb)
The following protocols were compiled by squad players at a meeting held on 4th August 2006. All players are required to adhere to these codes of conduct. Every player is required to complete this form.
Code of Conduct for Parents and Guardians Form (3kb)
All parents and guardians are required to abide by the North Hykeham Rugby Club Codes of Conduct. Please complete this form and show your support.
Permission to Attend Form (3kb)
Parental or Guardian consent is required to enable your child to attend training sessions and participate in matches for North Hykeham Rugby Club. This form also requests permission for coaching staff to administer first aid and make arrangements for medical treatment should this be required. Please ensure you have read the 'Players Protocols' and 'Code of Conduct for Parents & Guardians' before completing the form.
Health Check Disclosure Form (4kb)
North Hykeham Rugby Club has a duty of care towards its players. The Health Check Disclosure Form is a confidential document, and is used by the coaching staff to assess the ability of a player to train or play Rugby Union. Please complete this form as accurately as possible.
Youth Players Parental Media Authority Form (4kb)
North Hykeham Rugby Club requires parental or guardian permission for players to be included in future press and media articles. This may include written as well as photographic images, and will be undertaken in accordance with North Hykeham Rugby Club's child protection policy. In cases where authority is not provided, this will not prejudice your child's involvement in the youth section of North Hykeham Rugby Club.
Insurance Information and Player Declaration Form (6kb)
This form provides information relating to the insurance cover provided by North Hykeham Rugby Club on behalf of its players. The Insurance Information and Player Declaration form should be completed by all new youth players, and countersigned by a parent / guardian in the presence of a club official.
