Official DBS Continuation Sheet
The DBS have made it compulsory as part of the DBS application process for any additional information, such as addresses, previous surnames, criminal history etc. To be provided, with the DBS form, on the official DBS Continuation Sheet. Any additional information provided on paper or in a different format to this official sheet is likely to be rejected by the DBS - as such applications may be subject to delays.
