Official CRB Continuation Sheet
The CRB have made it compulsory as part of their new CRB application process for any additional information, such as addresses, previous surnames, criminal history etc. To be provided, with the CRB form, on the official CRB Continuation Sheet. Any additional information provided on paper or in a different format to this official sheet is likely to be rejected by the CRB - as such applications may be subject to delays.
